Under the supervision of the program Supervisor or their designee, the Case Manager provides services such as screening, support, brief intervention, collateral services, training, education and guidance to youth and families. Strength-based and trauma-informed support services are provided at various locations including in-home, at schools, and throughout the community. Case Managers are assigned to work in various Aldea programs including Probation Programs, Substance Use Prevention Programs, the ADAPT school program, and the Treatment Foster Care (TFC) Program.

**Duties and Responsibilities**:
**Essential Duties for Case Managers in Treatment Foster Care (TFC) Program**:

- _Provide behavioral management interventions to youth with trauma who are placed in foster care/adoption placements. _
- Provide in-home, in-community independent living skills training to youth and Non-Minor Dependents (NMDs).
- Provide case management assistance, including but not limited to scheduling medical/dental appointments, creating school and work schedules, etc.
- Document and communicate needs to the assigned Social Worker and participate in treatment meetings (e.g. CFTs, IEPs, TDMs) and discharge planning decisions.
- Provide youth/NMDs transportation to school, appointments, supervised visits, or court as needed.
- Supervise visits between foster youth/NMDs and birth family members and complete accompanying documentation.
- Provide crisis intervention services to youth/NMDs and family members as needed.
- Coordinate services and supports in conjunction with county and/or NMDs and Resource Parents and birth family members, including but not limited to coordination of therapeutic services, educational needs, family contacts, and other related activities.
- Maintain a flexible schedule to assist with emergency and/or urgent situations. Overnight/weekend assistance with clients may be required.

**Essential Duties for Case Managers in Prevention and Probation Services Programs**:

- _Provide prevention services in the community_, including in schools and at probation locations.
- _Conduct individual screenings, refer to treatment services when necessary, and facilitate individual and group services as indicated and completes all related documentation._
- _Actively utilize clinical supervision to develop and refine intervention skills and strategies._

**Essential Duties for Case Managers in the Aldea Day Adolescent Program for Treatment (ADAPT)**:

- _Supervise students, implement the behavioral management system, schedule activities, student’s implement academic and behavioral programs, enforce program rules and limit setting as defined by the program and treatment team, and monitoring students serving consequence and detention times._
- _Manage crisis situations in a calm manner, adhering to program and agency policies._
- _Communicate daily with the treatment team and consult regularly with Program Manager/Director._
- _Participate in program development and planning for special events._
- _Provide transportation for students and coordinate administration of medication, as needed._
- _Teach social skills, effective communication skills, and independent living skills to students._

**Agency Duties**:

- Develops and maintains good working relationships with external providers and stakeholders, clients and families, and colleagues. Maintains excellent boundaries with youth and families.
- Completes all required administrative activity, including clinical documentation, time tracking and billing backup record keeping in a timely and accurate fashion.
- _Actively participate in all agency required staff meetings, fundraising, community outreach and recruitment, and training opportunities. _Provides assistance and performs tasks related to program activities, compliance responsibilities, and special events.
- Maintains all current licensing and/or professional certification requirements and abides by all provisions of the Aldea Staff Handbook and agency and program-specific Policies and Procedures Manuals.
- Willing to perform other related duties needed as assigned.
