**Responsibilities as a MDS Coordinator**:

- Responsible for conducting and coordinating the development and completion of the resident assessment (MDS)
- Supervise the timely completion of each new MDS, and review/verify 100 percent completion of all sections of MDS by care conference date
- Assign assessment reference dates in conjunction with nursing and therapy
- Review and verify accuracy of each assessment to ensure that the appropriate RUG score is obtained
- Review all residents on admission for identification of level of care and to gather dat
- Maintain a Medicare log and conduct weekly Medicare meetings to review caseload
- Perform chart audits
- Demonstrates an understanding of MDS requirements related to varied payers including Medicare, Managed Care, and Medicaid
- Responsible for timely and accurate completion of Utilization Review and Triple Check
- Ensure timely electronic submission of all Minimum Data Sets to the state database
- Review state validation reports and ensures that appropriate follow-up action is taken
- Act as an in-house case manager demonstrating detailed knowledge of resident’s health status to develop an appropriate care pathway and timely communication of needed information to the resident, family, other health care professionals and third party pay

**Requirements**:

- Registered Nurse with current active license in NJ
- Minimum two (5) years of clinical experience in a health care setting
- Minimum of one (2) year of experience in a long-term care setting
- Prior experience as an MDS Director
