Housing Stabilization Case Manager

Pathway Homes is a nonprofit, community-based organization providing housing and supportive services to adults diagnosed with severe mental illnesses.

We are looking for **full-time Housing Stability Case Managers** in Fairfax County, Prince William County and the City of Alexandria. Our case managers help individuals experiencing homelessness transition into housing and develop the skills to maintain housing and strive towards achieving their full potential.

**YOU WILL**:
**Gain valuable experience with Pathway Homes’ proven recovery-based model for addressing severe and persistent mental illness.**

**Have a Monday-Friday schedule that is convenient and flexible. No weekends or holidays.**

**Work within a supportive environment meeting regularly with your supervisor and teammates.**

**WHAT WILL YOU BE DOING?**
- supporting individuals in successfully transitioning into Permanent Supported Housing from state hospitals, other supervised residences, or homelessness without disruption in their psychiatric stability or sobriety.
- assisting individuals with independent living, sobriety, psychiatric stability, housing stability and engagement in the community.
- supporting individuals’ ability to remain in housing by assisting them with adhering to lease terms, ensuring compliance with fire and safety standards, and developing positive relationships with neighbors, landlords and the community.
- advocating for, and supporting individuals in self-advocacy related to leasing, fair housing and reasonable accommodations.
- coordination of care for assigned individuals to include assistance with completing referrals to needed services and supporting the individual’s access to available community resources.
- Required qualifications_: Bachelor’s degree in social work, counseling, psychology or related field. Minimum one year experience working with individuals with co-occurring mental health and substance use disorders, and/or experiencing homelessness. Must have valid drivers license and reliable vehicle. Must be able to pass a background check and driving history check.
- Preferred Qualifications_: Community-based case management experience. Bilingual.

**Compensation** is $44,210 annually.
