Case Management Coordinator

**JOB SUMMARY**: Responsible to provide support to the Case Management/Social Work Department. This position will support all Case Management Department Staff by dispersing orders, processing requests for appeal letters, typing, filing, ordering supplies, payroll, data entry, answering the telephone and other duties that may be necessary to assist the department.
**PRINCIPAL DUTIES AND RESPONSIBILITIES**:Provides clerical support to the Social Work/Case Management Department which includes the social workers, case managers, financial counselors and department director. Distributes all requests for case management and/or social work services within timelines established by department procedures. Responsible for the daily count and distribution of the case manager’s cases, that are not working that day, and posting the number of cases each case working manager is responsible for. Post assigned cases for the financial counselors to ensure that the entire department is aware of who is responsible for which patients. Enters payroll hours in Kronos daily. Prints the various census reports and makes assignments as necessary (Medicare Observation). Perform as a Texas Notary Public for patients at St. Joseph Medical Center.
**MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED: WORK EXPERIENCE**:
Two years experience in a secretarial or administrative assistant position, required. Previous experience in a hospital setting, strongly preferred
**LICENSE/REGISTRATION/CERTIFICATION**:Texas Notary Public
**EDUCATION & TRAINING**: High School Diploma or GED required.
**SKILLS**: Technical, critical thinking, and interpersonal skills relevant to area in order to effectively communicate with physicians, health team members, patients and families, other health care professional and community agencies. Must be familiar with proper telephone techniques and possess excellent customer service skills. Ability to prioritize work with mínimal supervision, in order to independently carry out the duties of the position. Able to communicate effectively, in English, both verbally and in writing. Additional languages preferred. Basic computer knowledge. Proficient in using Microsoft office tools such as Microsoft Word, Microsoft Excel, and SharePoint.
