**Compensation Range: $17/hr
- $23/hr Commensurate with experience**

**Overview**:
The Hepatitis C Peer Navigator’s (PN) role is to assist individuals who have known Hepatitis C (HCV) infection and those at high risk with navigation and care coordination services.

Other activities include conducting outreach, providing harm reduction education, conducting rapid HIV and HCV testing, and coordinating medical and other supportive services as needed.

The PN provides group and individual HCV prevention education, HCV care coordination, and referrals to treatment and care.

The PN must be knowledgeable about community resources, including educational, social, and emotional supportive services available to clients.

Duties may also include recruiting clients, administering questionnaires, data gathering, maintaining patient databases, assisting with event organizing, performing data entry, program promotion, and group facilitation.

Conduct outreach activities including educational presentations, community hepatitis C rapid testing (including pre
- and post-test counseling), and facilitation of community linkage agreements.
- Coordinates follow-up testing and care for those who test positive.
- Performs rapid HIV and HCV testing.
- Builds relationships and locates resources that promote the recovery, independence, health, and wellbeing of service users.
- Conducts an initial assessment with each participant to assess their health and wellness status and experience with accessing health care.
- Conducts community health promotion, health education, and risk reduction counseling.
- Maintains routine communication with clients and assists with coordinating client’s care navigation goals.

Partners with agency staff and all members of the care team.
- Follows up with clients on a regular basis to keep them engaged and to assess progress.
- Navigates the medical system, including scheduling medical appointments, escorting clients to appointments, helping clients communicate with medical personnel, and assisting with any follow up as necessary.
- Completes and submits documentation of services provided in a timely and efficient manner.
- Participates in agency’s HCV events and/or initiatives as needed. Secondary:

- Attends required meetings as deemed appropriate and related to the scope of service.
- Adheres to workplace policies and procedures, including confidentiality, documentation, and conflict resolution.
- Participates in case conferences.
- Participates in quality assurance and quality improvement planning projects and activities as directed.
- Participates in monthly staff and peer worker development meetings and interventions.
- Conducts data entry and reconciliation for the program as needed.

Minimum Requirements
- High school diploma or General Equivalency Diploma minimum requirement
- New York City Department of Health Peer Certificate Training or the ability to obtain certificate through attendance at an AIDS Institute Authorized training center.
