**_The Health Home Care Coordinator is the lead office manager that provides administrative support to the Clinical Director, Program Supervisors, Administrative Director and Coordinator staff under the supervision of the Clinical Director of Care Coordination and the Administrative Director of Care Coordination. The Coordinator is responsible for the overall management of the Care Coordination office and operational supports. Coordinator maintains and communicates current and accurate care coordination data to all relevant staff and management to ensure maximum program operational efficiency. The Coordinator must be proficient in all related databases to include Foothold, HCS, GSI, EPACES, BTQ and SQL, and must demonstrate critical thinking skills in amassing and sorting data for compliance and reporting purposes. Coordinator manages and oversees all Care Coordination members' billing and provides support to SVW/ SJMC billing Department to ensure maximum monthly billing and favorable adjudication of claims denials. _**
