Hospital at Home- Registered Nurse

Do you have experience as an Acute Care RN? Join Brightstar Care in partnership with a leading hospital system to launch a groundbreaking Hospital at Home program. We are looking for RN's to provide high-quality, at-home acute care. The position requires acute care competencies, along with an ability to work independently in an at-home setting.

**Responsibilities**
- Utilize monitoring, assessment, planning, intervention and critical thinking skills to care for acute care patient populations in the home.
- Conduct home and audio/visual patient care visits as directed by the care team and/or protocol.
- Ensure all interventions in the patient's care plan are implemented efficiently with a patient centered approach.
- Adhere to the clinical protocols and be willing to obtain the necessary training to provide care within the context of the providing evidence-based care.
- Promote professional practice and a culture of safety; willingness to engage in process improvement efforts.
**Requirements**:
- Associate Degree required
- 2 years of Registered Nurse experience in an acute care setting. Home health experience preferred.
- Valid driver’s license, reliable vehicle and proof of vehicle liability insurance.
- Must be able to communicate effectively in English.
- Demonstration of core competencies will be completed within 90 days..
- Certified CPR and negative TB skin test or chest x-ray
- Ability to work on weekends, nights and holidays
- Reliable transportation
- Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the mobility of the client. Must be able to lift up to 50 pounds
