**Per Diem Saturday and Sunday 8:00am - 4:30pm**

**Remote position once trained**

**How You'll Contribute**:
Reporting to the Senior Director of Clinical Services the Triage Assistant will assist in the support of the day-to-day operations related to the clinical services of Care Dimensions patients, families, physicians and clinical staff. The Triage Assistant is an integral member of the hospice team, working closely with all IDT members to coordinate care needs for hospice patients and families. The Triage Assistant takes direction from the triage nurse or designee in regards to ordering appropriate durable medical equipment and clinical supplies for all hospice patients.

**What You'll Bring**:

- High school diploma required, Associate's degree preferred.
- Minimum of one year of experience in a medical facility or medical office setting.
- Proficiency in MS Office/Excel/PowerPoint/Outlook and healthcare specific systems and databases required.
- Ability to communicate clearly, orally and in writing and in a professional manner to coworkers, patients, family members and external customers.
- Ability to exercise sound and independent judgement.
- Ability to handle confidential information.
- Excellent organizational skills.
- The Triage Assistant is responsible to work, at minimum, two holidays per year.
