Case Manager Summary: Conducts the intake/admission and assessments; develops and implements the service plan; provides case management and re-entry/transition planning; primary contact for referral source; connects clients to community resources in collaboration with referring/assigned officer; liaison with various systems (court, social services, treatment providers, etc.); may need to attend court; engages and develops collaborations with community agencies; develops and submit required reports to referring/assigned officer and/or court; coordinates and/or facilitates groups and activities. Assist with screenings and as needed. May transport clients, based on programmatic needs. Must possess a valid driver's license. Expected to work some evenings and weekends. Duties: o Engages clients and uses Motivational Interviewing techniques in day-to-day interactions with clients. o Delivers gender responsive and culturally competent services (i.e. case management sessions, assessments). Schedule case management sessions. o Provides case management services in accordance with the Carey Guides. o Conducts approved assessments on clients. All assessments must be audio taped and submitted for quality assurance. Matches clients to programs and services based on client's risk, needs, strengths, and responsivity factors. o Develop Individualized Service Plans with respect to assessment findings and court ordered mandates. o Facilitate instructional and intervention group services. o Ensure security and confidentiality and uniformity of content and management of all client records. Provides timely and accurate file and database documentation. Keep records up to date at ALL times. Enters accurate data into Contractor Data Collection System. o Acts as a liaison to the court, referral sources, community resources and organizations. Represents program and Agency in a professional manner. o Remain current on research and literature of "best practices" for working with juvenile justice populations. o Maintain ongoing communication with referral agent. Prepares court reports, monthly reports and discharge reports that are accurate and timely. o Must attend trainings as set for by CSSD (Court Support Services Division) and meet with a quality assurance coach to review assessments and case management sessions monthly. Participates in quality assurance activities that include audio/video taping interactions

Pay: From $44,000.00 per year
