Care First Mobile Medical Group Inc is seeking a dedicated and compassionate Nurse Practitioner to join our team, specializing in providing healthcare services to patients in the comfort of their homes. As a Nurse Practitioner specializing in Home Health Services, you will play a crucial role in delivering comprehensive and personalized care to our patients.

**Responsibilities**:

- Conduct comprehensive patient assessments, encompassing medical history, physical examinations, and diagnostic tests.
- Diagnose and treat acute and chronic illnesses, injuries, and medical conditions.
- Develop and implement patient care plans collaboratively with the healthcare team.
- Prescribe medications and order appropriate laboratory and diagnostic tests.
- Provide patient education on disease prevention, treatment options, and healthy lifestyle choices.
- Perform minor procedures, including wound care, suture removal, and skin biopsies.
- Assist in the management of chronic diseases and conditions.
- Collaborate closely with physicians and other healthcare professionals to ensure optimal patient care.
- Maintain accurate and up-to-date medical records.
- Deliver services within the context of home health services, addressing the unique needs of patients in their home environments.

- Valid Nurse Practitioner license and certification in the state of [State].
- Proven experience as a Nurse Practitioner, preferably with a focus on home health services.
- Strong clinical and diagnostic skills.
- Excellent communication and interpersonal abilities.
