**Salary**:
Starting at $58,000.00 (Exempt)

**About the Position**:
Support the Care Coordination program throughout the agency service area. Provide support, information, and direction to care coordinators. Ensure program philosophy, policies, and procedures are implemented and enforced.

**A Typical Day as a Care Coordinator Supervisor**:

- Ensure that individualized services and support are provided in accordance with the unique potential and needs of each child and family.
- Evaluate all care coordination services provided to ensure the services are meeting long-term objectives of the agency and meeting standards of time and cost efficiencies.
- Review and approve records, reports, and documentation from supervised staff to ensure accuracy and compliance with guidelines and procedures.
- Provide weekly one-on-one supervision with care coordination team.
- Master's degree in Social Work, Marriage and Family Therapy, Psychology or related field
- Mental Health or Human Service Experience (preferred)
- Supervisor Experience (preferred)

**Additional Qualifications**:

- Completion of DCFS criminal background check and fingerprinting
- Must be at least 21 years of age
- Obtain an insured vehicle
- At least three years of driving experience with a valid driver's license
