Care Coordination Associate

**Position Summary**:
The Care Coordination Associate will function as an extension of the provider to help close the gaps in the patient's care for patients with complex medical needs; ensuring continuity of care and disease management for the patient. The C.A will build a rapport with patients through monthly calls to empower patients to meet their goals, guide them in their healthcare journey and advocate for their needs by utilizing active listening skills, identifying barriers, assisting with strategies to overcome barriers.

**Minimum Qualifications**:

- Highschool/GED Required. Associates degree in related area of study preferred.
- Medical Assistant certification from an accredited program required.
- 1-2 years of direct experience working with patients in a healthcare setting.
- Experience and proficiency working with electronic medical records.
- Ability to manage and support patient's healthcare needs, and complete individualized patient care plans along with performing care coordination.
- Excellent interpersonal skills.
- Computer skills, including ability to draft and create professional flyers.
- Bilingual Spanish/English is required.

**Key Duties**:

- Provides telephone support to patients which includes pre-visit planning, scheduling appointments, etc. etc..
- Develops an NCQA CCMR care plan with pts/family that includes self management goals, barriers, needs, interventions.
- Provides pt with referrals to health care team members including nurse care manager, SW, Health Educator, Nutritionist.
- Assist patients with management of care transitions.
- Adheres to documentation protocols and best practices for daily workflow, escalation of patient issues, and care plans.

**Work Environment Type, Location, and Schedule**:
Intermittent Hybrid, Bronx, NY, Monday - Friday 8am to 5pm or 8:30am-5:30pm.

**Starting Compensation**:
Minimum: $45,000 annually; Maximum: $47,000 annually
