**Position Summary**:
**Angelina County Champions for Families (ACFF)** is a dynamic project that provides local, research-based, innovative services to improve child health and development and assist families in achieving economic self-sufficiency and stability. ACFF aims to establish a sustainable, comprehensive community support system for vulnerable families. As a Family Navigator, you will play a pivotal role in the ACFF Project team, contributing to our mission by providing essential services, advocacy, and emotional support to families, promoting their overall well-being and resilience. This role offers a unique opportunity for professional growth and development, as you will be actively involved in the program’s success by connecting with families, providing essential services, and promoting well-being. Your work will involve accomplishing program/project goals and objectives as outlined in the grant, making a real difference in the lives of the families we serve.

**Education/Licensure**:
Master’s degree in social work (MSW) from an accredited program.Current and valid Texas state licensure as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW).One (1) year of paid experience in Public Health or Healthcare, specifically in providing social work services or equivalent.

**Essential Functions**:

- Family Assessment and Support:_
- Conduct comprehensive assessments of families’ strengths, needs, and challenges.
- Develop individualized service plans to address identified issues.
- Provide counseling, crisis intervention, and emotional support to family members.
- Case Management:_
- Coordinate services and resources for families, including healthcare, housing, and education.
- Advocate for families within the community and with other agencies.
- Monitor progress and adjust service plans as necessary.
- Parenting Education and Skill-Building:_
- Offer parenting workshops, classes, and support groups.
- Teach parenting skills, communication techniques, and stress management.
- Empower parents to create nurturing and stable home environments.
- Community Outreach:_
- Engage with community organizations, schools, and local partners.
- Raise awareness about program services and eligibility criteria.
- Collaborate with other professionals to enhance family well-being.
- Data Collection and Reporting:_
- Maintain accurate records of client interactions and service delivery.
- Prepare regular reports on program outcomes and participant progress.
- Ensure compliance with program requirements.

**Work Schedule**:
Works 7:30 am - 5:30 pm Monday through Thursday. Some weekends for community outreach events.

**Knowledge and Abilities**:

- _Clinical Competence:_
- Familiarity with evidence-based interventions and therapeutic approaches.
- Ability to assess mental health needs and provide appropriate referrals.
- Cultural competence and sensitivity to diverse populations.
- _Advocacy and Empowerment:_
- Strong advocacy skills to support families in accessing resources.
- Empower families to make informed decisions and navigate systems_._
- _Communication and Collaboration:_
- Excellent communication skills (verbal and written).
- Ability to work collaboratively with interdisciplinary teams.
- Proficiency in using technology for documentation and communication.
- _Ethical Practice:_
- Adhere to professional ethics, confidentiality, and legal standards.
- Maintain professional boundaries and handle sensitive information appropriately.
