**JOB SUMMARY**:
Community Mentors lead community programming, using volunteer sites, recreation and leisure activities and educational resources to develop personal growth, community engagement and job opportunities for associates with diverse abilities who have a goal of community employment.

**RESPONSIBILITIES**:

- Lead Community programming
- Prepare and deliver curriculum in topic areas of health, wellness, music, art, discovery, cooking, etc.
- May need to provide assistance with personal cares as defined by the associate’s plan.
- Document case notes showing progress toward goals
- Maintain awareness of associate’s interests, strengths, and needs to provide appropriate services and feedback during case review process.
- Use volunteer sites, recreation and leisure activities and educational resources to develop personal growth, community engagement and job opportunities.
- Redirecting the associates when exhibiting inappropriate behaviors.
- Transport and accompany associates to activities as needed in company vehicles.
- Maintain good verbal and written communication to ensure all parties are kept up-to-date and informed of any changes necessary relating to associate.
- Provide mobility training while in the community.
- Present and model soft skills training.
- Generate interest and familiarity with the community through supported experiences and engagement.
- Assist with identifying meaningful community resources.
- Provide supports in various levels/group sizes.
- Complete reports as directed.
- Respond to emergency situations promptly and administer First Aid or CPR/AED if needed.
- Report any and all accidents/injuries along with filling out reports within 24 hours.
- Always promote and maintain confidentiality of associates.
- Perform any other duties assigned by management

**REQUIREMENTS**:

- Ability to facilitate, initiate and respond to verbal, written, computerized, telephone and fax communications.
- Ability to demonstrate, train and/or model job tasks, duties, and responsibilities.
- Ability to perform quality inspection according to LPP’s and/or customer’s requirements.
- Mobility to access various locations within/outside LPP facilities.
- Attend meetings and training as needed

Pay: $15.50 - $17.00 per hour

Expected hours: 32 - 40 per week

- 8 hour shift
- Day shift
- Monday to Friday
- No weekends
