**About the Role**:
At Capstone Healthcare, we believe in the power of compassionate care and professional growth. As an LVN/RN, you'll play a vital role in providing person-centered care to our residents.

**Qualifications**:

- Graduate of an approved LVN/RN program with a state license.
- Minimum of 2 years nursing experience in a skilled nursing facility preferred.
- Proficient in care plan processes and electronic medical records.
- Strong attention to detail, documentation, and resident assessment skills.

**Responsibilities**:

- Facilitate patient care as directed by the attending physician, overseeing nursing care under the supervision of the DON.
- Assist in pre-screening, room assignments, and resident discharges.
- Supervise nursing activities and promote improvements in care quality.
- Maintain a comfortable, hazard-free environment and ensure adherence to safety protocols.
- Notify relevant parties of any new or unusual findings.
- Uphold confidentiality while fostering positive interactions with residents, team members, and visitors.
- Other duties as assigned.

**Physical Requirements**:

- Prolonged periods of standing and walking.
- Ability to lift, push, and pull up to 75 pounds.

Pay: $27.00 - $39.00 per hour

Expected hours: No less than 40 per week
