**Monday - Friday 2pm-10pm**

**JOB DESCRIPTION**

The RN/LVN will be assigned regularly to fill the Charge Nurse positions for 2pm-10pm, Monday thru Friday shifts in the facility, and supervise direct patient care personnel in the nursing unit. The RN/LVN reports to the Director of Nursing (DON)

**QUALIFICATIONS**:
Will be a RN/LVN who is a graduate of an approved school, and currently licensed by the Texas State Board of Nurse Examiners. It is desirable that the RN/LVN be trained or experienced in areas of supervision, rehabilitative, psychiatric, or geriatric nursing.

**DUTIES**:
Includes but are not limited to:
The primary purpose of the RN/LVN Charge Nurse position is to provide direct nursing care to the residents, and to supervise the day-to-day nursing activities performed be nursing assistants to ensure that the highest degree of quality care is maintained at all times.

Recognize significant changes in the conditions of patients and take necessary intervention.

Direct the day-to-day functions of the nursing assistants in accordance with current rules, regulations, and guidelines that govern the long-term care facility.

Ensure that the individual nursing care plan is followed.

Prepare and administer prescribed medications as needed.

Supervise serving of prescribed diets and fluid intake, takes note of deviations from normal and reports persistent unresolved problems to the physician.

Ensure that all medications and treatment are charted “after the fact” for the appropriate shift.

Notes and follows through with new and existing doctor orders.

Delegates tasks using good nurse’s judgement as allowed by license or certification.

Protect patients from accident and injury.

Prepare incident/accident reports.

Assure supportive care to prevent decubiti, footdrop or contractures.

Ensure that residents are clean and well-groomed.

Charting per his/her shift.

Responsible for notification of physician and family for resident concerns.

Perform treatments and evaluate resident response to treatments.

Obtain vital signs as ordered.

Complete nursing summaries as scheduled.

Ensure completion of flow sheets (to include I/O, BMs, meal intake etc.)

Ensure that clinical records or an abstract of them are forwarded with transfer of patients.

Ensure that drugs covered by Controlled Substances Act of 1970 are verifiable by inventory.

Participate in regular staff meeting and In-Services

Consult with the D.O.N. when necessary.

Teach and instruct nursing procedures to nursing assistants.

Perform functions as assigned by the D.O.N.

10 - 6 shift must be CPR certified.

Participate in professional and community endeavors and organization for betterment and improvement of health care.

Evidence interest in improving own skills and understanding through in-service and professional organizations, conferences, seminars, workshops, etc.

Promotes quality care to all residents.
