The CCHIP Transitional Corrections Specialist (TCS) will provide support to Adult and Corrections Clinic populations. The Transitional Corrections Specialist efforts will include communication and documentation of interactions with individuals living with HIV who are currently engaged in care. The Transitional Corrections Specialist will actively participate in the multidisciplinary clinic team and maintain professional and collaborative communication. The Transitional Corrections Specialist will assure that client is connecting to other core services, case management, dental, Mental Health and Substance Abuse treatment. They will assist with establishing and fostering relationships with community organizations that service the correctional population. The Transitional Corrections Specialist will work closely with Cermak Health Services to set a transitional care plan with clients released until engagement in care at any CCHIP Site or HIV primary care center. They will also provide benefits counseling to clients and assist in educating, enrollment, verification, and utilization of those benefits. Serves as an advocate for the correctional population attempting to engage in care

SPECIFIC RESPONSIBILITES

The TCS will assist in completing initial intakes and assessment of needs on all new clients and clients returning to care.

Make appropriate and timely referral to internal and external providers and coordinate services.

Identify and address barriers and/or challenges to care and services.

Remind patients of upcoming appointments, provide navigation assistance at initial adult care appointment, and encourage participation of clients in clinic events.

Provide education and information on the transition from corrections to adult healthcare.

TCS will outreach corrections no shows and assist in engaging clients back into medical care and social services.

Communicates with the multidisciplinary care team about individual patient needs and concerns.

The TCS will attend clinic, pre-clinic meetings and client centered staffing’s.

Complete training curriculum that includes skill building and career planning to promote self-empowerment, retention, viral suppression, and HIV testing.

Shares knowledge and education to improve patient’s knowledge and attitudes toward treatment adherence and social supportive services/activities.

Establishing and foster relationships with community organizations that service the correctional population.

Work closely with Cermak Health Services to develop a transitional care plan that begins at release and ends at engagement in care. Staff might be required to gain access to Cermak Jail and visit facility for engagement in care.

Provides Health Literacy to all clients which will help navigate clients through the HIV and primary care system.

Adhere to Social Services Documentation Policy of all Patient Encounters and Forms.

Maintain a working knowledge of community and internal program that would enhance the client’s ability to be maintained in care.

Adhere to all mandatory CORE Center/CCH-CCHIP/AFC-Corrections mandatory trainings and meetings.

Adhere to CORE Center, Social Services & AFC Specific Policies & Procedures

Provides clinic coverage/general coverage when a shortage arises.

Other duties as assigned by Supervisor.
