**CASE WORKER / COMMUNITY TRANSITION ADMINISTRATIVE ASSISTANT**

**FULL TIME / 35 HOURS PER WEEK**

**BASIC JOB FUNCTION**, this position supports the Community Transition Liaison to operate at the top of their role and responsibility. Provides additional support to the Liaison for complex cases. Ensures case documentation meets the standards set forth by the Executive Office of Elder Affairs for the Community Transition Liaison Program.

**QUALIFICATIONS**: High School diploma with one year of related work experience and/or interest in working with nursing facility residents. Excellent documentation skills, organizational skills and ability to meet deadlines. Strong attention to detail and accuracy. Ability to work with a positive attitude in a team setting. Excellent computer skills required, including thorough knowledge of Microsoft Word, Outlook, and Excel. Must have a valid driver’s license and provide own transportation.

**SALARY**: $46, 239
