**Job Summary**:
Asserting and safeguarding the human civil rights of mentally and physically challenged persons and their families and fostering human dignity and personal worth of each individual. Performing as a member of the clinical interdisciplinary team, acts as a community liaison, and provides clinical and/or concrete services.
**Job Qualifications**:
1. Masters Degree in Social Work from an accredited program and NYS licensure in Social Work (LCSW or LMSW) and one (1) year experience working with the mentally and/or physically disabled.
2. Requires knowledge of the principles and methods of social work.
3. Maintain valid NYS driver’s license within agency criteria required.
**Essential Functions**:
1. Functions as a member of the interdisciplinary and clinical team.
2. Facilitates admission process consisting of intake process and intake review committee.
3. Responsible for all required documentation; progress notes, 30 Day reviews in accord with Medicaid Waiver status, DDP 1, DDP 2, etc.
4. Provides individual and group counseling to both individuals and family members when necessary.
5. Assists in arranging for the use of other community resources and acts as a liaison between the individual and the community.
6. Provides consultation to other staff to aid in preventing, identifying, and treating personal and social problems of individuals.
7. Intervenes in crisis situations when necessary.
8. Facilitates various activity groups.
9. If employees holds a LCSW (Licensed Clinical Social Work) license may be required to review and approve Behavioral Support plans developed by agency BIS’s which include restrictive techniques or are considered intrusive plans to ensure plans meet regulatory standards. Will collaborate with Behavioral Services Coordinator for all additional plans
Pay: $34.29 per hour
