We are currently seeking a Per Diem Registered Dietician to provide nutritional services to our patients within a multidisciplinary team approach. We are looking for someone who is capable of being understanding toward our patients with unique needs.

**Responsibilities**:

- Conduct nutrition assessments of home care patients.
- Formulate nutritional plan for therapeutic diets based on medical, physical condition, lifestyle, culture and nutritional needs.
- Provide ongoing support and community resources to patients and care givers as needed.
- Participate in appropriate long-term planning in anticipation of patient discharge from Agency services.
- Complete clinical notes for each contact accurately and timely.
- Maintain compliance with all state and regulatory bodies along with Moorestown VNA policies and standard operating procedures.

Qualifications:

- Current NJ State Registered Dietician License.
- One year of work as a Registered Dietician in a health care setting.
- Home health experience preferred but not required.
