**Position Summary**:
As a Family Partner (Case Manager) at Project HOPE Boston, Inc., this role reports to the

Family Partner Manager, operating within a comprehensive support network for families providing trauma-informed advocacy, prevention, and intervention services. This position requires attention to detail, compassion, the ability to manage approximately 30 - 35 cases simultaneously, and accessibility. Clients may call, walk in, or be referred, and their information and updates must be tracked and reported regularly with ongoing stabilization support for up to

12 months. This role requires establishing a robust support network and coordinating services to promote transitional support and family welfare.

**Agency Background**:
Project HOPE Boston, Inc. has provided support to families in the Nubian/North Dorchester area for over 40 years, always with a laser focus on partnering with families seeking housing stability and economic mobility. Since its inception, Project HOPE has implemented a number of innovative and impactful programs to address housing, workforce, childcare, and educational needs.

Located in Boston’s Dudley Street neighborhood, Project HOPE Boston, Inc. is at the forefront of efforts in the city to move families beyond homelessness and poverty. It was founded in 1981 by the Little Sisters of the Assumption, who first settled in the Dudley area in 1947 to live and work with families in the local neighborhood. In the 1980s, when family homelessness was identified as a crisis in Boston and the state, the Sisters opened their doors. They welcomed families to stay with them, and Project HOPE (House Open People Enter) Boston was established. Once best known as a family shelter, Project HOPE today offers an array of services to assist families in gaining the resources and skills that can lead to more opportunities and access. Partnering with families in neighborhoods that have been historically marginalized,

Project HOPE centers its efforts and work on the Social Determinants of Health with a racial equity lens.

In 2006, Project HOPE Boston, Inc. opened an attractive Community Building on Dudley Street
- the first LEED Silver certified, earth-friendly building in Roxbury - to serve as its headquarters and a neighborhood center for job training, adult education, housing counseling, community empowerment, and more. Just a few blocks away, the original site remains home to the family shelter.

**Key Responsibilities & Expectations**:

- Conduct an intake assessment with each family to obtain relevant information, including but not limited to housing, education, employment, legal, health, and childcare needs.
- Collaborating with provider agencies and services to identify and support families referred for prevention services, including but not limited to housing, education, employment, legal, healthcare, and childcare needs.
- Managing a caseload of 30 - 35 families assigned by the supervisor.
- Implementing personalized re-housing plans and facilitating access to vital resources
- Advocate for each family according to their needs and goals. This may include accompanying families to their Dept. Of Housing & Livable Communities (HLC) appointments, legal services, housing searches, etc.
- Regularly research and keep current on all community resources, make referrals, and follow up to ensure these resources are in place.
- Meet with the family regularly at established intervals to ensure active participation in implementing the plan.
- Coordinate all review meetings with shelter staff, Housing Search Specialists, education and training programs, or others as needed.
- Enter all case notes, assessments, and other touchpoints into Project HOPE databases.

- Support families with all legal, employment, and financial concerns that may impede housing search, including assisting in CORI clearance and working with other organizations and resources to obtain deposits and additional initial housing fees.

**Administrative**:

- Serve as primary liaison to DTA, HLC, legal services, and all other relevant referral resources and agencies.
- Maintain all documentation in compliance with state and federal program regulations.
- Work with the Family Partner Manager to ensure all resident procedures and policies are documented and followed for all monitoring purposes.
- Maintain all assessment tools, reports, and files as Project HOPE Boston, Inc. requires.
