Provides direct nursing care to residents in their homes. Provides direct and indirect nursing services within the scope of LPN practice as defined by the Arizona State Board of Nursing and the established policies and procedures of the agency.

**ESSENTIAL RESPONSIBLITY**:

- Intervention and modification of the plan of care.
- Assist other health care personnel in the delivery of patient care.
- Maintains the environment of care including equipment and other material resources.
- Provides emotional and physical comfort for patients.
- Observes, records, reports, and acts upon changes in the patient’s condition.
- Performs nursing activities.
- Participates in professional development.
- Identifies clinical or operational performance improvement opportunities and assists in continuous quality improvement.
- Orders, prepares, and documents medications and treatments as prescribed by the physician.
- Provides and documents triage emergency home visits when a resident needs assistance.
- Follows-up on laboratory results and subsequent physician orders.
- Documents nursing care in the clinical record on a timely, accurate basis.
- Assists with resident activities of daily living, including personal care, wheelchair escorts, and medication reminders.
- Fills medi-sets,updates medication administration records, clinical records, and medication records.
- Assists in health promotion activities such as influenza and blood pressure screening clinics.
- Performs other related as assigned.

**EDUCATION/EXPERIENCE**:

- Current unencumbered LPN license from the State of Arizona.
- CPR and First Aid Certification.
- Minimum 5 years’ experience as a LPN.
- Previous home health or retirement living experience preferred.

**PHYSICAL DEMAND**:

- Frequent walking, standing, bending, lifting, and moving intermittently.
- Carry at least 20 pounds.
