This position provides close coordination with the Housing Authority of Cook County (HACC) Property Manager to identify individuals and families in need of community resource education, linkage to crisis services and Integrated HealthCare Services as needed.

**ESSENTIAL DUTIES ANDRESPONSIBILITIES** include the following. Other duties may be assigned.

Initiate and coordinate referral to Grand Prairie Services Intake Department.

Advocate on client’s behalf for access to community resources.

Promote an active participation in decision making and self-advocacy in all aspects of services and recovery.

Provides community support services to clients in a manner to ensure linkage and access to necessary resources and services.

Provide linkage case management, counseling and other necessary resources when appropriate.

Participate in scheduled team meetings.

**EDUCATION and/or EXPERIENCE**

Bachelor’s degree (B.A.) from four-year College or university in psychology, counseling, or social work. Meets eligibility requirements as a Mental Health Professional as defined in Medicaid Rule 132. Must have proficient typing and computer skills.

**LANGUAGE SKILLS**

Ability to read and interpret documents such as policy manuals, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of clients or employees of the organization.

**MATHEMATICAL SKILLS**

Ability to add, subtract, multiply, and divides in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

**REASONING ABILITY**

Ability to define problems collect data, establish facts, and draw valid conclusions.
