The Clinical Supervisor, in collaboration with the Clinical Director, is responsible for the management of clinical personnel to include RN Case Managers, Hospice Aides, Spiritual Counselors and Social Workers. This professional will oversee new program development, while focusing on the maintenance and improvement of quality end of life care and services.

ESSENTIAL JOB FUNCTIONS:
1. Responsible for the development and compliance of the agencies Infection Control program.

2. Responsible for the development and compliance of the agencies Wound Care policy and protocol.

3. Implements and maintains cost effective wound care.

4. Reviews and evaluates each case through a variety of means such as home visits, conferences, record review and the services provided by clinicians; discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality hospice care services; is available at all times during operating hours to assist clinicians as appropriate.

5. Assists clinicians in establishing immediate hospice goals and development of plan of care, as needed.

6. Assure attendance to care conference meetings held by external healthcare entities, with hospice personnel to facilitate coordination of care and discussion of interdisciplinary group involvement.

7. Assists in the screening and interview process of new employees and makes recommendations for employment.

8. Assists in the orientation of new employees as assigned.

9. Assists Clinical Director and hospice Medical Director in the planning, implementation and evaluation of in-services and continuing education programs.

10. Assists in formulating, revising, implementing, and evaluating organizational policies, procedures, goals, and objectives, both short and long term as it applies to clinical services.

11. Participates in the establishment of a competency based clinical program.

12. Manages/leads weekly interdisciplinary team meetings.

13. Provides annual required Hospice Aide classroom training.

14. Timely completion of initial, 90-day, quarterly and annual evaluations and competencies of direct reports and those assigned by Clinical Director.

15. Tracks time and attendance of direct reports and executes disciplinary action in accordance with agency disciplinary policy in collaboration with Human Resources.

16. Coordinates and leads APEX orientation for any contracted clinical personnel.

17. Responsible for organization of level of care changes in accordance with policies and Medicare regulations.

18. Develops and leads training of new programs and services, in accordance with healthcare trending and assessment of consumer need.

19. Maintain competency tracking for educational purposes for clinical personnel.

20. Leads the agency Safety Committee.

21. Manages and performs audit checks on admission binders for patients’ in-home and in facilities.

22. Rotates as AOC (administrator on call).

23. Assists CEO and Clinical Director with complaint investigations.

24. Manages all clinical personnel’s initial, quarterly/annual competency.

25. Provides one on one meetings no less than quarterly with clinical personnel.

26. Assigns/delegates admissions, new and updated

27. Participate in the development and execution of annual clinical Key Performance Indicators.

28. Oversight of Service Intensity process.

29. Participates in meetings/committees to include Ethics, Management, Emergency Preparedness, QAPI, and Policy /PIP Reviews.

30. In collaboration with the Clinical Director, provides in-person competency and field visits for RN’s no less than once a year.

31. Participates in the annual program assessment and business plan.

32. Follows agency complaint policy and process.

33. Complies with all Health Insurance Portability and Accountability Act (HIPAA) requirements in accordance with federal, state, and organizational policies.

34. Provides other duties and activities as delegated by the Clinical Director.

REQUIRED SKILLS, ABILITIES AND KNOWLEDGE:
1. Assess, evaluate, and respond to patient situations quickly, providing emergency care in crisis as quickly as possible.

2. Ability to work independently, while collaborating with other team members and managers.

3. Works with diverse social and economic situations without imposing own values on patient or family.

4. Maintains appropriate professional boundaries.

5. Excellent written and verbal communication and public relation skills.

6. Organizational skills.

7. Demonstrates effective communication and listening skills.

8. Proficient in the use of electronic medical records and other programs, as necessary.

9. Knowledgeable in hospice state and federal regulations to include Conditions of Participation

10. Knowledgeable in Anti-Kickback Statute.
