**Position Overview**:
The Hospice Community Liaison builds relationships with healthcare providers, community organizations, and individuals to promote hospice services and facilitate referrals. Serving as a liaison between the hospice organization and the community, they educate key stakeholders about the benefits of hospice care, contributing to the organization's growth and mission of providing compassionate end-of-life care to individuals and their families. **Must have prior hospice marketing experience.**

**Key Responsibilities**:

- **Relationship Building**:_
- Cultivate ties with healthcare professionals (physicians, nurses, social workers, discharge planners) to promote understanding of hospice services and facilitate referrals.
- Partner with community organizations (senior centers, nursing homes, assisted living facilities, religious institutions) to raise awareness of hospice care and foster collaborations.
- Represent the hospice organization at networking events, conferences, and community meetings to build connections within the community.
- **Referral Management**:_
- Be the main contact for healthcare professionals and community members seeking information about hospice care.
- Promptly respond to inquiries, offering accurate information about services, eligibility, and the admission process.
- Coordinate with the admissions team to ensure a smooth transition for patients from referral to admission.
- **Education and Awareness**:_
- Deliver presentations, workshops, and educational sessions to healthcare professionals, community groups, and individuals to enhance understanding of hospice care, eligibility criteria, and available services.
- Provide information and resources to aid informed decision-making about hospice care for healthcare professionals and community members.
- Collaborate with the marketing team to craft educational materials, brochures, and presentations that communicate the benefits and value of hospice care.
- _**Be able to successfully recruit 3-5 qualified hospice patients per month.**_
- **Market Analysis and Planning**:_
- Monitor market trends, competitor activities, and community needs to identify growth and improvement opportunities in hospice services.
- Collaborate with the leadership team to develop strategic plans and initiatives enhancing the organization's presence and impact in the community.
- Provide community feedback and insights to inform program development, service enhancements, and marketing strategies.
- **Documentation and Reporting**:_
- Maintain accurate records of interactions with healthcare professionals, community organizations, and individuals.
- Prepare regular reports on referral trends, outreach activities, and educational effectiveness.
- Utilize customer relationship management (CRM) tools or relevant systems to track and manage relationships, referrals, and outreach activities.

**Qualifications**:

- Associate or bachelor’s degree in healthcare administration, marketing, business, or a related field, or relevant experience.
- Previous experience in sales, marketing, or community outreach, preferably in healthcare or hospice care.
- Strong communication and interpersonal skills to build rapport and establish professional relationships with a diverse range of stakeholders.
- Excellent presentation and public speaking skills to communicate complex information to different audiences.
- Knowledge of hospice care, end-of-life services, and the healthcare industry.
- Familiarity with referral management processes and the ability to track referrals efficiently.
- Strong organizational and time management skills to prioritize tasks and meet deadlines.
- Proficient computer skills, including CRM tools, Microsoft Office, and other relevant software.

Pay: $75,000.00 - $90,000.00 per year
