RN Hospice

**(full-time, direct hire position with excellent full benefits package and competitive base/bonus compensation structure)**

**Responsibilities**:

- Completes initial comprehensive and ongoing assessment of patient and family to determine Hospice needs
- Admissions of patient for services and development of the initial patient/family Plan of Care
- Coordination of services with the patient, family, and hospice interdisciplinary team members
- Ongoing clinical supervision of Licensed Practical Nurses and Home Health Aids

**Requirements**:

- Valid Registered Nurse (RN) license
- RN Diploma, Associates or B.S. degree in nursing
- Minimum 1 year of Hospice agency experience required
- Valid Driver’s License, auto insurance and reliable transportation
- Excellent observation, problem solving and communication skills

**Job Types**: Full-time, Permanent

Pay: $96,000.00 - $101,000.00 per year
