**The Nile Health Network operates a significant Group Residential Community-Based Services program throughout the Phoenix area and Statewide. We provide caregiving services, more specifically habilitative and attendant care supports to those with developmental disabilities. We provide training and continuing education, resources and supports to our Wellness Coaches from the start of employment and throughout. **We have several opportunities available and are looking to fill shifts immediately. Shifts and work schedules vary per client’s needs.**

**Summary of DCW Position for Developmental Group Homes**

**Provide individualized and quality services to individuals within their home living environment and community. Instruct by individualized service plans (ISP), provide support in the areas of daily living, leisure skills, planning community involvement activities and promoting health and wellness education. Present necessary information at ISP meetings and act as liaison for individual/family concerns as necessary. Must be available to respond to calls for service and support day, night, evening, weekends, and holidays. Job requires being willing to travel.**

**Essential Functions**:

- **Assist in the maintenance of departmental operations to achieve compliance with contract requirements, accreditation, licensing and monitoring standards, agency procedures, Federal/State/Local regulations and all other licensing/regulatory agencies as directed.**
- **Adhere to current training requirements and applicable accreditation and licensing/regulatory standards and compliance with the Health Insurance Portability and Accountability Act (HIPAA) and Health Information Technology for Economic Clinical Health (HITECH) requirements.**
- **Coordinate program information and contribute to the successful resolution of challenges. Maintain relationships with agency departments, families utilizing or inquiring about services, individuals receiving services, staff, upper management, and community.**
- **Ensure that services provided are person centered, incorporate active treatment, are developmentally appropriate, culturally sensitive and show respect and dignity.**
- **As requested, present necessary information at ISP meetings, and act as a liaison for individual/family concerns as necessary.**
- **Ensure the accurate and timely completion of administrative tasks as assigned to include, but not limited to payroll, individual support plan (ISP) documentation, case record documentation, individual attendance, individual’ s spending money, medication administration, and general record keeping. Attend meetings and actively participate in internal and external committees as assigned.**
- **Provide mentoring, training, guidance and support the program’ s preferences and priorities as related to the mission statement.**
- **Transferring individuals from wheelchair to changing table, bed, chairs, shower chairs etc.**
- **Assisting individuals in and out of vehicles.**
- **Group home cleaning which may include but not limited to sweeping, mopping, vacuuming, dusting, and laundry. Assist with the planning of monthly menus of well-balanced meals/snacks meeting applicable dietary needs and restrictions, as required.**
- **Assist with person hygiene which includes, but not limited to showering, changing adult incontinent briefs, brushing teeth, shaving, and assisting with dressing. Advise, consult, and coordinate with applicable program staff in all departments.**
- **Familiar with and adhere to all department Standard Operating Procedures.**
- **All other duties as assigned.**

- **Twisting/turning.**
- **Visual and auditory acuity (if applicable).**
- **Maintain body equilibrium.**
- **Finger dexterity.**

**Equipment Used**:

- **Van wheelchair lifts, including tie downs; (if applicable).**
- **Ability to become knowledgeable of all types of wheelchairs; (if applicable).**
- **Operating Hoyer lifts, lifting devices and gait belts; (if applicable).**
- **Household appliances, (i.e., stoves, washers, dryers, dishwashers, vacuums, microwaves, appliances; (if applicable).**
- **Computer usage (i.e., Share Point, E-mail, Boosted, Kronos, Internet, tablets, cell
