The Social Worker position assists and supports patients and their families to identity areas of need. Works with the Case Manager of inpatient services to assist patients and their families

**ESSENTIAL JOB DUTIES**
- Assists with discharge planning primarily in Acute Care.
- Work with IDT to identify needs.
- Discuss/arranges options, services, and resources to help with areas of need.
- Assist with Health Care Power of Attorney documents and other Advanced Directives as able.
- Complete needed, required, and professional documentation and paperwork for documentation of patients' charts and other jobs tasks and requirements.
- Perform other duties as requested.
- REQUIRED CERTIFICATION

CPR certification

**Job Types**: Full-time, Contract

Pay: Up to $58.00 per hour

Schedule:

- 8 hour shift
- Day shift
- Monday to Friday

Work setting:

- In-person

**Experience**:

- Social Worker: 1 year (required)

License/Certification:
