- term residents including memory care. Our team of seasoned staff work together to offer excellent service and care to each resident.

The Registered Nurse Assessment Coordinator (RNAC) is responsible to coordinate fulfillment of regulations relative to the MDS and associated forms, coordinate the interdisciplinary Plan of Care, to ensure appropriate utilization of services relative to levels of care, and to assist the Lead RNAC in ensuring that the highest degree of quality care is maintained at all times for each of the residents.

Registered Nurse Assessment Coordinator (RNAC) Responsibilities:

- Reviews current Medicare charts on a regular basis to ensure documentation compliance of Medicare Regulation
- Coordinates Plan of Care with clinical team
- Ensures Pain assessments and CAAs care plans are addressed
- Utilization Review in Medicare NOMNC/ABN Letters, Medicare Certifications, hospital referral review, evaluation of hospital records for ICD10 coding, setting of ARD dates, etc., of admissions and readmissions, OBRA scheduling, Part B Authorization as needed, CMI Picture Date
- Coordinate information with other departments when coding MDS
- On-going education for initial RNAC certification and recertification requirements every 2 years to keep up with all updates and changes in MDS coding
- Takes direction from Regional RNAC and is able to support recommendations to work as a team
- On-going education of caregivers regarding MDS coding monthly
- Assist in implementing the established orientation program for new employees where appropriate
- Develop and maintain a good working rapport with interdepartmental personnel, as well as other departments within the facility to assure that nursing services and activities can be adequately maintained to meet the needs of the residents
- Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment
- Maintain the confidentiality of all resident care information
- Participation in weekly CMI meetings
- Performs other related job duties as required

Registered Nurse Assessment Coordinator (RNAC) Qualifications:

- Must have a current RN license in the Commonwealth of Pennsylvania RNAC Certification
- Must have MDS experience
