Health Care Coordinator

**Position Summary**:
Provides health care coordination to include primary care, psychiatric and medication coordination for consumers of Turning Leaf. Ensures compliance with county, state and accreditation bodies standards. Ensures communication and collaboration between physicians, TL management and TL clinical team with regard to general care coordination for all consumers. Provides communication across, within and between all players and functions providing health maintenance services to the consumer.

**Qualifications**:
Two or more years of experience with MI/DD/TBI populations and/or Adult Foster Care experience; management experience highly preferred. Certified Clinical Medical Assistant (CCMA) required. Proficient in computer skills to include the Internet, Electronic Medical Record (NextStep/CareSuite/QM), Scheduling Software; Microsoft Word, Excel and Power Point.

**Compensation**:
Negotiable, commensurate with experience and education

Pay: $18.00 - $22.00 per hour

Expected hours: 40 per week
