**Position Summary**

Under the coaching and guidance of the Program Manager, the Family Support Coordinator provides client support through S4KF’s system of wraparound values, support services, and case management to HIPPY participants who are experiencing the need for additional resources to be successful as a parent s identified at assessment or during services. Resources could be in several areas including but not limited to basic needs, mental health, substance abuse or coordination with schools.

The Family Support Coordinator shares in S4KF’s responsibility for adherence to all organizational policies and procedures, as well as compliance with all applicable program, federal, state and local laws and regulations. He/she must be able to represent S4KF in a highly professional manner, ensure the confidentiality of clients, and work effectively with families, children and community liaisons.

**Principal Duties and Performance Standards**

**Service Provisioning**
- Collect referrals for support coordination from the HIPPY Home Educators and determine needs
- Complete a community based, limited needs assessment with the family
- Complete the Perceived Stress Scale with each family to assist in determining appropriate resources
- Conduct family consultation to develop a family support plan
- Facilitate family team meetings including formal and in-formal reports
- Refer for services and follow-up with family and service providers on those services;
- Review and update family support plan as needed
- Monitor effectiveness of services and re-refer to services as needed
- Provide collaboration and consultation services for home educators and other team members:
**Organizational Support**
- Coordinate, provide and/or recommend training to HIPPY Home Educators and other team members, when necessary, on topics related to behavioral health and working with families with challenging issues;
- Develop relationships and/or MOU’s with providers to facilitate effective referral of HIPPY families to services;
- Conduct or participate in provider meetings to maintain positive relationships with community service providers.
- Assist in ASO budget development when appropriate
- Follow the data collection guidelines provided by the Children’s Board of Hillsborough County
- Document all client activities in S4KF health record
- Provide other support as required.
- Promote excellence and continual improvement by participating in the organization’s Performance and Quality Improvement Process.

**Training Requirements**
- Attend HIPPY annual conference when funding is available
- Shadow HIPPY Home Visitor to gain information about the program and services
- Complete all required S4KF trainings including attending quarterly training days.

**Knowledge, skills and Abilities**
- Abide by all S4KF, Healthy Families Hillsborough and Healthy Families Florida policies and standards of conduct.
- Effectively communicate with a variety of populations; peers, clients, client family members, service providers, perspective employers, etc
- Able to identify applicable community resources.
- Knowledge of confidentiality regulations, procedures and information exchange agreements to protect information from unauthorized disclosure.
- Ability to render compassionate and equal care to all program participants regardless of gender, race, religion or sexual orientation.
- Must be able to maintain effective and professional boundaries with staff, community partners, and program participants.

**QUALIFICATIONS**
- Minimum of a Bachelor’s degree in social work, counseling, psychology, nursing or a related field
- Minimum of two (2) years’ experience providing referrals and case management services to vulnerable populations
- Bi-lingual (Spanish/English) preferred
- Able to build and maintain the community’s trust for S4KF as an effective, expert and compassionate provider of services.
- Able to pass a DCF Level II criminal background screen and a pre-employment drug screen
- Ability to travel as required by the position.
- Must have a valid Florida driver’s license, and a reliable transportation.
- Required to obtain, maintain and provide proof of automobile insurance coverage reflecting the following coverage amounts:

- $25,000.00 Bodily Injury Per Person
- $50,000.00 Bodily Injury Per Occurrence
- $25,000.00 Property Damage

**WORKING CONDITIONS AND PHYSICAL REQUIREMENTS**
- This position is both an office and field position. The position’s office will be co-located with the Healthy Families teams that they are supporting.
- This position will require non-traditional hours including evenings and weekends.
- Spend some time driving and getting in and out of the car to see clients in their homes or at school.
- Able to see clients under conditions that may be considered uncomfortable or even hazardous, including but not limited to poorly maintained premises that may lack ventilation and/or air conditioning, kept untidy, dirty,
