We are looking for a compassionate Hospice Chaplain to join our growing team. In this position you will be providing care in patients’ home or a facility where they live. As a Chaplain, you will be providing bereavement and spiritual support to patients and families by offering education, support, and resources. As part of the interdisciplinary team, you will be involved in the plan of care as well as assistance of final arrangements and or discharge planning.

**Responsibilities**:

- Provide spiritual services to help patients and families to understand the dying experience and give support in coping with changes taking place
- Provide spiritual support, counsel, prayer and ministry as appropriate
- Maintain communication with spiritual leaders and community chaplains to provide extra spiritual needs
- Attends IDG meetings regularly and implements the Hospice Plan of Care.
- Assist with community awareness functions, as necessary.

**Requirements**:

- 2 years of active work in spiritual care ministry and pastoral experience
- Ability to work with patients and families of various denominations/beliefs
- BS or BA in theology, religion, human services, counseling or psychology from a credited college or university
- Valid drivers license with maintenance of good driving record and reliable transportation

Pay: $60,000.00 - $65,000.00 per year

**Experience**:

- Hospice care: 2 years (preferred)
