**DaVita is looking for a Licensed Social Worker (LMSW) to support our patients and their families at our Melrose** Dialysis Clinics in the Bronx, NY**

- ** Coverage: Covering Melrose Dialysis Clinic **_(459 E 149th St. Bronx, NY 10455)_**:

- ** Monday - Friday schedule / 8 AM - 5 PM**:

- ** Full time schedule - 40 hours**:

- Life on dialysis can be a difficult transition for many patients-and you'll be there to support and advocate for them.
- Help patients understand their rights and responsibilities, and guide them in managing the physical, mental, emotional and financial demands of End Stage Renal Disease.
- Build meaningful and long-term relationships with patients and their families in an intimate outpatient setting.

**Requirements**:
Licensed or registered in the state of practice as required by state regulations
- LMSW or LCSW
- Meets all state required regulations to practice in the dialysis setting.
- Master's degree in Social Work (MSW) required with a specialization in clinical practice
- Demonstrated knowledge of government and private insurance programs
