The Health Services Coordinators oversees and has a primary responsibility for managing, planning, implementing, and evaluating comprehensive health, nutrition, mental health, and disabilities services for children and their families enrolled in the Home-Based Early Head Start (EHS) program. Intentional interaction with home visitors, HB supervisor, parents, pregnant women, and consultants. Lead program Health Services Advisory Committee. Additionally, this position will collaborate and work closely with the EHS staff to plan integrated services for all enrolled children, families, and pregnant women.

The Health Services Coordinator serves as the supervisor of the Disability and Wellness Specialist. Assisting Disabilities and Wellness Specialist with training needs, building program capacity, and monitoring program progress.

**Responsibilities**:

- Coordination and/or completion of all health histories at enrollment, vision, and hearing screenings within the 45day period, and other health related services as needed for newly enrolled children.
- Collaborates with HB Supervisor to ensure access to consultants are aligned to the needs of the home visitors.
- Oversight of the health, nutrition, disabilities and mental health content areas to ensure adherence with HSPS & EHSN policies and procedures and implementation of the OHS Health Services Competencies.
- Ensure all enrolled children are up to date on the schedule of age-appropriate preventive and primary health care needs (i.e., well-baby checks and immunizations). Work collaboratively with home visiting staff to inform any needed medical, dental, and immunizations, or nutritional needs are met and/or followed up upon.
- Ensures parent consents and the release of information for all health, mental health, dental, and nutritional services provided to children by the program or its contracted service providers are received and on file.
- Hold the responsibility of health, nutrition, mental health, and disabilities services and inform parents in writing of the results of all services.
- Develop relationships in the community with clinics, doctors, and dentists that are frequently used by parents and/or in the community that may be able to service families in the program to manage relationships and assist families as needed, with securing a source of accessible health care for their children/family. In collaboration with Doula Manager, develop written MOU agreements that form partnerships for related services.
- In collaboration with the Site Director, identify Nutrition Consultant to support the program's nutrition services and coordinate the schedule of the nutrition visits ensuring at a minimum of eight to ten hours of service per year.
- In collaboration with the Doula Manager and Program Director, ensure that consultants (nurse, Mental Health professional, and dietitian) provide services to children, families, and staff as needed or requested.
- Be up to date on changes or updates to state or federal health/nutrition requirements along with current health/nutrition trends affecting prenatal and birth-5 population served. Provide information related to health, dental, nutrition and mental health topics to home-based staff, and families.
- Organize and facilitate the activities of the agency's Health Services Advisory Committee (HSAC); ensure the Committee membership is composed of all required professions and parent representation and that it meets at least twice a year.
- In conjunction with the HB Supervisor develop written health, sanitation, and food service policies and procedures ensuring compliance with the Head Start Performance Standards, and other local and state regulations. Ensure these policies and procedures are carried out including scheduling and conducting required facility checks and monitoring.
- As required, maintain detailed, accurate, current and confidential records including child health files, health services tracking system and documentation of all health activities (create monitoring system). Ensure accuracy of ChildPlus records.
