We are a **SUCCESSFUL Home Health Organization** located in the Chicago Land area We are hiring a Home Health RN (Registered Nurse) to conduct **patient home visits in the Western Suburbs**

Join our growing team of healthcare professionals as a home health nurse
Home Health Nurse RN Responsibilities:
- Completing nursing process in delivery of patient care which includes: assessment, planning, implementation, and evaluation of patient outcomes.
- Total oversight of patient from admission to discharge.
- Developing plan of care based on findings.
- Serving as patient’s advocate and maintaining ongoing communication with physician.
- Timely completion of all skilled nursing documentation
Qualifications:
- Current Illinois State RN License
- Valid Drivers License and Reliable Vehicle
- **MINIMUM of 2 years experience as an RN**:
- Must be RELIABLE and DEPENDABLE
- VALID CPR CERTIFICATION
- Must be COMPASSIONATE when engaging with patients in their homes.
**Job Type**: Part-time
Pay: $80.00 - $90.00 per hour
