Hope Hospice & Healthcare, Inc., a leading provider of end-of-life care, is seeking a dedicated professional full-time Hospice RN (Mon-Fri, 5d x 8h) to join our team. The Hospice RN will manage a case load and assist with admissions as needed.

Our purpose is to ensure the highest quality of life for our patients and their families by providing compassionate care, comfort, dignity, support, and hope. We are just **1 of 2 hospice agencies in all of Southern CA **to be designated by CMS (Center of Medicare Services) as a 5-Star Quality Hospice Agency, and currently in our 4th consecutive quarter achieving this rank.

**General Responsibilities include but are not limited to**:

- Coordinates the services provided to his/her patients.
- Works cooperatively with the attending physician, hospice Medical Director, and other members of the interdisciplinary team.
- Participates in patient care conferences and supervises the hospice aides assigned.
- Regularly reviews and updates the plan of care.
- Performs prescribed medical treatments, including pain management and symptom control.
- Conducts assessments and evals, maintain excellent oral and written communication skills.
- Provides education and supportive care to patient and caregiver.
- Current CA RN License.

**- **Minimum of one-year experience as a professional hospice nurse within the last 3 years
- Valid driver's license, clean DMV record, auto insurance and dependable transportation required.
- BSN is preferred. Certification in Hospice and Palliative Nursing is desired.

Pay: $45.00 - $50.00 per hour
