HE LOVES TO COOK FOR HIS FAMILY AND WOULD LIKE SUPPORT IN LEARNING AND COOKING MORE RECIPES AT HOME. HE CAN WALK TO COMMUNITY VENUES AND ALSO DRIVES, BUT PREFERS TO HAVE SOMEONE WITH HIM FOR SUPPPORT WHEN ACCESSING THE COMMUNITY FOR SHOPPING, GOING OUT TO EAT, VISITS, ERRANDS, APPOINTMENTS AND MOVIES. THE PARTICIPANT IS LEGALLY BLIND IN HIS LEFT EYE AND REDUCED VISION IN HIS RIGHT EYE THAT IS BEING MONITORED. HE WEARS GLASSES. NICHOLAS ENJOYS GOING SHOPPING, ESPECIALLY AT GAME STORES. HE ENJOYS GOING OUT TO EAT OR ORDERING TAKE OUT. ONE OF HIS FAVORITE PLACES IS CHIPOTLE.ALLERGIC PERFUMES, COLOGNES, CANDLES, AXE DEODORANTS, FORMALDEHYDE, MERCURY, AMONIA, PINE SOL. AIR FRESHENERS.**

HE USES HIS WALKMAN -HE NEEDS PROMPTS FOR TYPICAL HOUSEKEEPING TASKS AND NEEDS TO USE NON- TOXIC CLEANING PRODUCTS. PREFERRED ACTIVITIES INCLUDE PLAYING INSTRUMENTS, VIDEO GAMES, MOVIES AND LISTENING TO HIS FAVORITE -NEEDS DIRECTION AND ENCOURAGEMENT-SUPPORT IN DEVELOPING AND MAINTAIN TYPICAL FRIENDSHIPS, - INCLUDE OPPORTUNITIES FOR GETTING TOGETHER. **ESSENTIAL DUTIES AND RESPONSIBILITIES **i**nclude the following. Other duties may be assigned.**
- **Provide one-on-one and person centered -focused, direct services to the participant in his Everyday Life.**:
- **Work collaboratively with his Support Team to develop and implement the outcomes, goals, needs and desires in the Individualized Support Plan (ISP).**:
- **Provide additional direct services as needed which may include assistive technology (AT), instrumental activities of daily living (IADL) assistance, self-advocacy training, and other staffing supports as determined on a daily basis.**:
- **Assist with identifying service gaps and unmet needs of cos.**:
- **Collaborate with community partners and resources to identify participants**
**Provide opportunities and choices for community participation for all individuals supported. Maintain current training required for the provision of services.**
**REQUIRED**
**Knowledge and/or Experience**
**Knowledge of the educational needs of participation directed services Knowledge of the principles and techniques of effective supportive communications; Knowledge of the principles, methods and techniques of supporting someone in the community with triggers and sensory issues.**
