**Mental Health Professional**

The Mental Health Professional provides direct patient care to patients who are identified to have a mental health and/or substance use illness.
- Provide direct, field based services to all patients who are diagnosed with a mental and or substance abuse illness.
- Provide both individual interventions, such as brief individual counseling, case management, referral and linkage to social services; utilize a variety of evidence-based practices in interventions
- Assist in conducting both individual needs assessments
- Functions as a member of a multidisciplinary clinical team and interprets clinical information for use in diagnosis and treatment.
- Establishes and maintains effective relationships with contractors and community organizations and other governmental agencies regarding programs and services; participates in committees, meetings and conferences; provides guidance regarding appropriate social-psychological practices and services.
- Makes referrals to government and community resources for appropriate level of care or additional resources.
- May be required to perform crisis interventions.
- Follow the ethical and practice guidelines as outlined by the Board of Behavioral Science
- Actively support the achievement of CalOptima Health’s vision and goals for its Street Medicine Program as defined in the Street Medicine Operations Manual
- Engage with providers and patients to create a trusting environment
- Complete all required documentation in multiple computer systems including care plans and progress notes within 24 business hours of the service occurring.
- Maintain a good working relationship with all staff on the team.
- Attend and actively participate in all CalOptima Health led meetings
- Participate in case conferences
- Knowledge of and ability to maintain HIPAA requirements.
- Knowledge of and/or ability to learn basic technological skills (Word, Excel, PowerPoint, Outlook, EMR) and open to new technology
- Drive medically equipped van as needed.
- Other projects and duties as assigned.
- Possession of a Doctorate or Master's Degree from an accredited U.S. graduate school, or a certified foreign studies equivalency in social work, psychology, counseling or a closely related behavioral science field.
- Must be currently registered as an Associate Clinical Social Worker (ACSW), Associate Marriage and Family Therapist (AMFT), Associate Professional Clinical Counselor (APCC), Licensed Clinical Social Worker (LCSW), a Licensed Marriage and Family Therapist (LMFT), or a Licensed Professional Clinical Counselor (LPCC) with the California Board of Behavioral Sciences.
- Licensure/registration must be maintained as a condition of employment. Failure to maintain licensure/registration may result in immediate termination.
- 1 year of experience working with severe and persistent mental illness and/or substance use disorders required
- Experience working with community-based organizations a plus
- Experience working with unsheltered individuals a plus
- Experience working with community-based organizations a plus
- Adaptable and open to change and problem solving
- Excellent customer service skills required
- Cultural competence and sensitivity required, multilingual skills a plus, especially Spanish or Vietnamese
- Ability to:

- maintain patient confidentiality required through knowledge of HIPAA requirements

- Knowledge of:

- Mental health diagnoses
- Efficient interviewing practices and techniques used to gather information
- Proof of a valid driver's license in good standing
- Have access to a vehicle.

Pay: $75,000.00 - $82,000.00 per year
