**Program Coordinator - HIV Community and Population Health - Grant Funded**

Prevention Program Coordinator promotes and supports patient access to Comprehensive Status Neutral Sexual Health services including Screening/Testing and Treatment, Pre-Exposure Prophylaxis (PrEP), Post-Exposure Prophylaxis, HIV Testing, and HIV primary care linkage and linkage to support services. Educate potential patients about PrEP/PEP and encourage consistent sexual health and primary health care follow up visits. The Program Coordinator will Provide adherence counseling for clients receiving PrEP/PEP or Antiretroviral treatment. Program Coordinator will conduct community outreach to spread community awareness of HIV Prevention and access to care. Coordinator is responsible for Patient Panel Management; coordinate support with patients to attend follow up appointments and access to support services, addressing social determinants to health and wellness. As well as assist patients with benefit/Insurance eligibility and navigation.

**This is a grant funded position.**

**Preferred Criteria**
- Bilingual Spanish
- Experience with program planning and evaluation and contract management
- Experience working with LGBTQ community, youth & adolescents, persons experiencing homelessness, as well as immigrant and under documented populations
- Additional experience: data recording in SharePoint, REDCap eSHARE, EPIC, AIRS a plus

**Required Criteria**
- Bachelor's Degree or 3 -5 years of equivalent experience
- Proficiency with MS Office
- Excellent writing and communication skills - possess verbal and written skills to convey mission and purpose to internal and external stakeholders.
- Strong interpersonal skills, highly organized, ability to work with diverse community groups

$58,500-$79,000
