**Per Diem Administrative Nurse Evaluator (RN)**

**Reports to**: Director for the Nursing Home Transition Diversion and the Traumatic Brain Injury Waivers, and the Assistant Directors (Leads) for each Waiver.

**Travel Required**:Yes, across both Nassau and Suffolk County -The Administrative Nurse Evaluator will have office space at SILO in Medford, N.Y., but assessments may be conducted telephonically or virtually from the Administrative Nurse Evaluator’s home. Assessments may also take place at the client’s home should an in-person assessment be warranted.

**Reliable car is a must.**

**Salary: $150 per UAS (annual cap of $30,000)**

**Applicant will be trained**

**Qualifications**:
**License or Certification(s) -**
- A current New York State licensed Registered Nurse.
- **Prefer** New York State certified to complete a Unified Assessment System (UAS) or be able to complete the certification within two months of hire.
- To be qualified to be the Nurse Evaluator for SILO’s Regional Resource and Development Center the individual must:

- Have two years’ satisfactory professional experience and dedicated commitment to work with individuals with disabilities and/or seniors in a community setting.
- Excellent clinical assessment skills
- Excellent communication and interviewing skills
- Excellent problem-solving skills and ability to resolve conflicts
- Working knowledge of services and supports available through Medicaid, Medicare and/or other third-party payers.

**Responsibilities**:
The Administrative Nurse Evaluator’s (RN) sole responsibility will be to assess individuals identified by the Department of Health (DOH) and as requested by the Director of the Waiver Programs as needing a Uniform Assessment System-New York (UAS-NY) of which must be submitted in a timely manner. Administrative Nurse Evaluators may also be requested by the Director and/or Assistant Director(s) to participate in Team Meetings as well as Fair Hearings on an as needed basis.

Pay: Up to $150.00 per hour
