Case Manager responsibilities include:

- Assist individuals to locate and access the services outlined in their Plan of Care to remain in their home
- Evaluate health, welfare and service need options for independent community living
- Assist consumers with goal setting and provide resources to remain in the community
- Work with individuals and their families to locate agency staffing or direct their own care
- Offer training and support to individuals who choose to direct their own care

**Required Qualifications**

Case Management/Care Coordination Agency staff shall meet the following qualifications:

- four years of education in a health or social services field and one year of community experience: or
- a license in social work or social services field; or
- a license in nursing or related health care field; or
- a registered occupational therapist licensed to practice in the State of Maine; or
- a certified occupational therapy assistant who is licensed to practice occupational therapy in the State of Maine under the documented supervision of a licensed occupational therapist.

**Salary and Benefits**:
Starting Salary commensurate with experience: $64,000 - $67,000.

Summary of Benefits:

- 12 Paid Holidays
- Earned Time Off (ETO - vacation and sick days), beginning with 3 weeks and 3 days ETO per year in the first two years and increasing thereafter.
- Health Insurance-Alpha One pays 90% of the insurance plan cost for individual employee plans
- 401(k) Plan: Employee becomes eligible to join the Plan the first of the month following 3 months of employment; Alpha One provides an Agency Match equal to 100% of first 3% of salary contributed by the employee and 50% of the next 3% of salary contributed by the employee, the highest “safe harbor” benefit permitted under Federal law
- Family Leave for the birth or adoption of a child: 12 weeks paid leave

**We are currently working from home and have returned to in-person visits for case management in the home of consumers.**
