**LOOKING FOR FLEXIBILITY IN YOUR WORK SCHEDULE?** Interim HealthCare is a **New Hospice Agency operating in California City, CA looking for a Per Diem Registered Nurse to join a family-oriented company, o**wned and operated by Saint Andrew’s Abbey.** We are an upcoming hospice company looking for a dedicated hospice RN case manager to join our family-oriented team who is empathetic, hardworking, compassionate, and a dedicated team player. We offer competitive hourly pay plus mileage reimbursement. Pay for patient care/visit hours, and pay for hours spent charting $45 - $55 per hour depending on experience. **If you are wanting to have more control over your work schedule, come see us.**

**Essential Functions**:

- Performs assessments and procedures, which require substantial specialized knowledge, judgment, and nursing skill based upon principles of psychological, biological, physical and social sciences.
- Visits in home clients to evaluate the quality of care being provided and reviews clinical notes.
- Completes nursing initial and ongoing assessments of all in home clients within specified deadlines.
- Participates in ongoing interdisciplinary assessment of the patient.
- Participates in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
- Obtains physicians plans of treatment and orders, and initiates their execution.
- Provides services that are ordered by the physician as indicated in the plan of care.
- Counsels and educates the patient/client and family in meeting healthcare related needs.
- Effectively communicates with other members of the interdisciplinary healthcare team providing care to promote coordination of patient/client care and planning for discharge.
- Secures equipment and supplies and coordinates placement for use in the home.
- Participates in Quality Assessment and Performance Improvement (QAPI) program.
- Participates in in-services or training, as requested.

**Minimum Education & Experience Requirements**:

- Graduate of an accredited school of nursing.
- Licensed to practice as a RN in California. License is active and in good standing.
- 1 year of accumulated experience as a registered nurse within the last five (5) years.
- _One (1) year of hospice care experience preferred, but not required: willing to do admissions, continuous care, supervisory visits, PRN and routine visits._
- Able to provide proof of valid driver’s license if applicable.
- Able to provide proof of valid auto liability insurance if assignment(s) include driving vehicle.
