Hospice Chaplain/bereavement Coordinator/volunteer

Summary:
The Chaplain provides spiritual support to the Hospice Program. Spiritual needs are assessed and direct ministry to patients and families provided through spiritual guidance and counseling. Duties require an informed respect for the belief system of others.
Major Duties and Responsibilities:
Spiritual/Pastoral counseling services are provided in accordance with the plan of care and include:
- Spiritual/ Pastoral assessment as indicated and appropriate is completed as soon as possible after admission to
- Direct ministry, counseling and guidance to hospice patients and their families in accordance with on-going wishes and needs as soon as possible after admission to hospice and at the time of death when
- Develops an individualized spiritual/pastoral plan of care which:
- Demonstrates an effort to work in close collaboration with local clergy whenever desired by the patient/family.
- Provides spiritual support as needed and defined by the patient/family.
- Offers patients/families of different philosophies and religious beliefs opportunities to discuss and share their thoughts, feelings, beliefs,
- Documents spiritual assessments, care plans for future patient intervention, and on
- going
- Works with other professionals in resolving spiritual/pastoral/ethical
- Works with other members of the interdisciplinary hospice team to evaluate patient/family response to
- Bereavement visits to the patient/family prior to death and availability to family during the bereavement period if needed.
- Meeting with individual hospice staff regarding personal spiritual/pastoral issues that may affect their ability to function effectively
- Miscellaneous:
- Presides at hospice memorial
- Participates in orientation of new staff and
- Provides education to staff and
- Conducts wakes & funeral services when
- Refers patients, families and staff to community agencies as appropriate
- Serves as a consultant in spiritual concerns to the bereavement
- Participates in department QAPI
- Accesses sufficient hours of continuing education necessary to maintain
Qualification Requirements:
Is employed full-time by the hospice or has a contractual arrangement that provides the comprehensive spiritual/pastoral component of the hospice.
- Education and/or Experience:_
Bachelors degree in a humanity such as social work, pastoral counseling, nursing, sociology or philosophy. Master of Divinity, Social Work, Nursing or Pastoral Counseling preferred
Required License, Certification and/or Registration:
Certification through Clinical Pastoral Education. Valid Maryland driver’s license, current automobile insurance and availability of a motor vehicle
**Job Type**: Part-time
Pay: From $30.29 per hour
