***: The Case Planner is responsible to manage an average caseload of about 12 to 15 families at any given time. The Case Planner will conduct home visits and meet with families to assess the families' strengths, the safety, risk and well-being needs of children and the assistance that families may require. The Case Planner reports directly to the Supervisor of ACS. The Case Planner is expected to effectively communicate, provide appropriate documentation as outlined below and adhere to deadlines as set forth by the Director of Prevention Family Services. The Case Planner is responsible for providing and coordinating excellent quality care and healing to UAU’s participants and ensures that every one of the children on the caseload is living with a nurturing and safe, environment. This position must maintain professionalism in all areas, including confidentiality.

**PRIMARY JOB RESPONSIBILITIES (include but are not limited to)**:

- Conducting safety and risk assessments for families at risk of losing their children to foster care
- Maintain a caseload of approximately 12 to 15 families
- Create individualized service plans for each family focused on reducing assessed risk
- Support families with connecting to community resources, financial entitlements, mental/physical health services, educational supports, etc.
- Provide casework counseling in supporting families toward reaching their service goals
- Maintain consistent communication and appropriate contact with families including scheduling and conducting monthly in-home visits.
- Attend offsite meetings such as in-home visits, school visits, mediation meetings etc.. and as directed by the Director of Prevention Family Services and Supervisor of ACS.
- At times, as scheduled, must be able to attend required meetings at the preferred time of families and/ school staff.
- Participate in weekly one on one meetings with the Supervisor of ACS.
- Complete and provide applicable proper documentation from direct supervision and record accordingly based on department protocols, procedures and policies.
- Timely completion of documentation including assessments, case contact notes, etc. Including maintaining an accurate family appointment/field visit calendar.
- Attend training, continuing education conferences and department related conferences as approved and directed by the Director of Prevention Family Services.
- Properly and accurately follow protocols, related to case practice management and consultation; specific practice models include but not limited to Family Support Programs, Mobility Mentoring and Solution-Based Casework.
- Perform other duties as assigned.

**JOB QUALIFICATIONS**:

- Bachelor’s Degree in either Arts, Science or Social Work, required
- 2 years minimum of relevant child welfare experience, required
- Excellent communication skills, both verbal and written.
- Strong computer/technology skill set, including proficiency with Microsoft Office Suite (Word & Excel), CONNECTIONS and PROMIS.
- Ability to work in a collaborative environment and open to accept constructive criticism.
- Obtain and maintain all security clearance and training requirements throughout the duration of employment.
- Must be culturally competent - the ability to understand, be sensitive to the needs of, communicate with, and effectively interact with people across all cultures.
- Familiar with the regulations and requirements for ACS and NY State Departments of Health and aware of the compliance requirements as enacted by Federal, State and local jurisdictions as applicable based on the department’s contractual obligations.
- Required; Valid drivers license and reliable, accessible mode of transportation
