**DEFINITION**

Under general supervision, develop treatment plans and coordinate the care of nursing home residents. Once hired, employees may have the opportunity to advance to the Senior level after meeting the advancement criteria specified in the Employee Benefit Handbook.

**EXAMPLES OF DUTIES**

Complete resident interviews and summarize the information. Provide expertise in mood, behavior, and psychosocial well-being of the residents. Facilitate meetings with the resident, resident representative and interdisciplinary treatment team. Ensure that residents and resident representatives have a voice in the care planning process including developing resident centered goals and interventions. Monitor and document residents’ response to interventions and make updates to ensure their needs are met. Assist staff in understanding resident individual care plan interventions through education, role modeling, and individual coaching. Provide ongoing supportive contacts with residents that include education, empathy, and therapeutic interactions to address challenging behaviors. Manage resident personal trust account in cooperation with resident and financial decision maker (or payee) to meet resident needs. Provide professional testimony at court hearings. Perform other duties as assigned.

**EMPLOYMENT STANDARDS**

**Education, Training and Experience**:
Applicants with a Bachelor’s or Master’s Degree in Social Work will be screened through. Applicants who are currently in their final semester of school and are obtaining a Bachelor’s or Master’s Degree in Social Work will also be screened through. Applicants with any other major must possess a training, temporary or permanent Social Work Certification from the WI Department of Safety and Professional Services (DSPS) prior to applying.

Social Worker II

All employees hired with a Master’s degree and six (6) months of relevant experience or a Bachelor’s degree and two and a half (2.5) years of relevant experience will be hired as a Social Worker II, range 20. Those who do not meet these criteria upon hire will be hired as Social Worker I and will advance to Social Worker II after obtaining one of these criteria.

Requires a valid driver's license (or eligibility for), access to reliable personal motor vehicle transportation, and a good driving record. Must possess a Cardiopulmonary Resuscitation (CPR) Certification within one year in order to pass probation.

**Background Check Statement**:Some positions may require a criminal background check which can include fingerprinting due to the nature of the job's responsibilities. Wisconsin's Fair Employment Law, s. 111.31 - 111.395, Wis. Stats., prohibits discrimination because of an arrest or conviction record. 1) Provide services in both English and the required foreign language (e.g., Spanish, Hmong, etc.), including oral interpretation and written translations in that language.
