Our Primary Care Nurses (RN) utilizetheir extensive education and training to provide personalized care to ourpatients in a rewarding environment that promotes autonomy.
**Job Description**:
- Perform comprehensive assessments to establish anindividualized care plan.
- Facilitate communication between the patient and theinterdisciplinary team.
- Develop and maintain a therapeutic relationship by providingeducation and promoting self-advocacy and independence.
- Utilize state-of-the-art technology to document skilledservices provided.
**Qualifications**
- Registered Nurse with Valid CT RN license
- One year of RN experience; Homecare experience preferred.
- Valid driver’s license and reliable transportation required.
- Computer proficiency required.
