Overview:
**Responsibilities**:

- Provide direct patient care in accordance with established policies, procedures, and protocols
- Collaborate with healthcare professionals to plan, implement, and evaluate patient care plans
- Administer medications and treatments as prescribed by the physician
- Monitor and assess patient conditions, communicate changes to the healthcare team
- Maintain accurate and detailed medical records using electronic health record systems like Epic and Athenahealth
- Ensure compliance with HIPAA regulations and maintain patient confidentiality
- Participate in sterile processing procedures when necessary

**Skills**:

- Demonstrated experience in case management and patient care coordination
- Proficiency in medical coding practices
- Knowledge of ophthalmology procedures is a plus
- Familiarity with Athenahealth and Epic electronic health record systems
- Experience in hospital or nursing home settings preferred
- Strong understanding of HIPAA regulations and patient privacy laws
- Ability to work effectively in a fast-paced environment while maintaining high-quality patient care
