Haven Health, the leading Home Health, Palliative, and Hospice Company in Southern California, is currently hiring an LVN Intake Coordinator to fill our position within our Hospice division.

**Responsibilities**:

- Receives and processes intake data from the Agency’s referral sources:

- Review H & P
- Verify MD orders
- Communicate with Case Manager, MD, Patient and DME as needed.
- Update Patient Pending Board
- Ensures that all client needs are accurately documented on the proper forms.
- Fills all open orders.
- Assigns appropriate field staff.
- Answers call from field staff.
- Complete all tasks assigned (Continuous Care, Routine Visits, PRN Visits, etc)
- Collaborate with Interdisciplinary Team (Physician, Nursing, Social Services, and Chaplains) on a Plan of Care individualized to patient

**Requirements**:
Qualifications:

- Spanish Speaking required.
- Current California Licensed Vocational Nurse
- Hospice, Home Health, Public Health experience a plus
- Excellent written and verbal communication skills
- CA licensed and insured driver with reliable personal transportation
