Registered Nurse

**Creekside Post-Acute has been serving San Jose, California for over 50 years**. It is a 130-bed that specializes in short-term rehabilitation and skilled nursing. We are looking for a committed, caring, and reliable **Registered Nurse** to join our Skilled Nursing Facility
- Location: 3580 Payne Ave, San Jose, CA 95117, United States
- Position Type: Per Diem - NOC Shift
- Pay Range: $50.00 - $60.00
- Department: Director of Nursing

**RESPONSIBILITIES**:

- Administer medication and monitor patient responses
- Collaborate with interdisciplinary healthcare teams to create and implement individualized care plans
- Assess and document residents' health status regularly
- Provide guidance and support to Certified Nursing Assistants and Licensed Vocational Nurses
- Respond promptly and effectively to emergenciesCollaborate with physicians to ensure proper coordination of care
- Educate residents and their families on healthcare needs and provide emotional support

**REQUIREMENTS**:

- A valid and active California license as a Registered Nurse is required
- Must have an active BLS through the American Heart Association
- Must have an active CPR Certification
- Is in good standing with all regulatory agencies and licensing boards is required
- Graduate of an accredited school of nursing
- Interest in working with the geriatric population
- Ability to communicate effectively with residents, families, and healthcare professionals
- Strong organizational and time-management skills
- Compassionate and patient-centered approach to care
- Proficient in medical documentation and record-keeping
- Ability to work independently and as part of a team

**EXPERIENCE**:

- New Graduate or Newly Passer are Welcome
- Experienced Registered Nurse (RN)
- Interest in working with the geriatric population
