**DESCRIPTION OF DUTIES/ESSENTIAL FUNCTIONS**
- **
I**nterview patients and record their medical history using various electronic systems; verifies medical history.
- Perform biometric screening such as blood pressure, temperature, pulse, A1C, and lipid panels.
- Administers and issues medications and vaccines according to approved standing delegation orders.
- Performs patient follow-up; assist Chief Nurse as directed.
- Assist in compiling and entering data for reports.
- Maintains inventory of supplies and medications and cleanliness of the office/center.
- Assist to ensure positive outcomes in all clinical service areas.
- Participated in quality improvement activities including record reviews, chart audits, and skills assessments.
- Conduct health education and health coaching, training, and health promotion programming including providing specialized health information, knowledge and skills on diabetes, hypertension, COVID-19, and chronic diseases.
- Establish and maintain close working relationships with internal staff, community-based and health sector organizations to coordinate and perform health promotion activities, and community outreach.
- Perform other duties as directed.
- This site is eligible for APRN/RN Loan Repayment Program

**WORKING CONDITIONS**
**The position routinely requires lifting of moderately heavy items, such as typewriters or record boxes (up to 40 pounds) and/or very long periods of walking on rough surfaces on a routine basis.**

**MINIMUM REQUIREMENTS**:
**EDUCATION REQUIREMENTS**
Requires a high school diploma or GED certificate and graduation from a school of vocational or practical nursing.

**LICENSE / CERTIFICATION REQUIREMENTS**
Must be licensed or eligible to practice as a Vocational Nurse in the State of Texas. CPR certification required. License and CPR certification must be maintained and current for continued employment.

**EXPERIENCE REQUIREMENTS**
One (1) year of nursing experience as a licensed vocational nurse is required.
Two (2) years of experience as a Certified Medical Assistant in a public health or related clinical setting may be substituted for the experience requirement.

**PREFERENCES**:

- Valid Texas Driver’s License and compliance with the City of Houston's policy on driving (AP 2-2).
- Directly related professional experience or training in Diabetes or geriatric care preferred.
- Preference for applicants that are bi-lingual in English and Spanish based on community needs.
- Experience using Microsoft Office products including WORD, EXCEL, POWERPOINT, OUTLOOK.
- Experience delivering one-on-one patient education and group presentations preferred.
